Payment Options

Insurance and Billing Information

CHO accepts most insurance plans and can work with patients on an individual basis. We request you have your current insurance subscriber card with you at each visit so that we may keep your information up to date and file your claims efficiently  For insured patients, we must request payment of your co-pay amount at your visit. We accept the following health care providers:

 

· Medicare & Medicaid

· BCBS

· United Healthcare

· Aetna

· Multiplan Network

· Cigna

· Medcost

· Tricare

· Most Workers Compensation insurance carriers

 

For patients without insurance, we request payment at the time of service to balance your account and avoid billing expenses. For patients with financial hardship, we will work with you regarding a payment plan. Our business office can answer other questions you may have about insurance and payment plans.

 

Workers Compensation

If  your appointment is related to workers compensation, please inform us when you make your appointment. Please bring all required documents and authorizations for treatment upon arrival for your appointment.  

According to state law we are required to respond to any requests made by your employer for records.  We may request a reasonable fee for these responses to cover our expenses.

 

 

 Patients Directed to our Office by the Emergency Room

If you were injured and received treatment at the Durham Regional Emergency Room and were told to follow-up at our office but you do not have insurance or any method for payment, please inform the receptionist. If you have an urgent need for follow-up but no insurance and no method of payment, please discuss this situation with the receptionist to make special arrangements.
