Prescription Refills

We require written authorization to provide copies of your records. Please provide our office with a release of information form that we will fax or mail to you. With your authorization we will send progress notes after each visit to the doctors and/or attorneys involved in your care. If you are covered by worker's compensation, as part of the regulations, your case manager will have access to our medical records. This keeps all the interested parties well informed. Medical Records Request

Requests for prescription refills can be called into our office any time. We will respond the next business day. For urgent requests please call before 3:00 PM.

Please include the following information: when you request a refill:

· Your name

· Phone number where we can reach you

· Medication name

· Telephone number for the pharmacy
